The Wisconsin Physical Therapy Association cordially invites you to attend the 2011 Spring Conference – Extraordinary People / Innovative Ideas at the Heidel House Resort and Spa in Green Lake.

All classes are held on a first-come, first-served basis. Sign up early!
Conference registration includes continental breakfasts, morning and afternoon breaks, luncheons, Welcome Reception, Business Meeting, Awards Dinner (Pre-Registration required) and conference materials.
Confirmation will be sent upon receipt of registration.
Registration deadline is April 4, 2011. After that, plan on registering on-site with an additional $20 fee ($10 for students).
Refunds less a 25% administrative fee will be given if requests are received prior to April 1, 2011. No refunds will be given after April 1, 2011. All refunds will be issued after the completion of the conference.
Attendees will receive contact hours for courses attended in their entirety. These contact hours are Class I as defined by the Wisconsin Department of Regulation and Licensing. See course descriptions for specific contact hours.

It is the position of the Wisconsin Physical Therapy Association that course material is directed toward an audience of physical therapists, physical therapist assistants and PT/PTA students. Participants may only use the course material if it falls within their scope of practice. In addition, elements of the course used for patient/client management may only be defined as physical therapy when performed by or under the supervision of a physical therapist in accordance with American Physical Therapy Association.

A block of rooms have been reserved at the Heidel House for registrants and their guests.

Rates are $119 single/double (each additional person beyond double occupancy is 15.00.)

Be sure to make your reservation by contacting the Heidel House at 800/444-2812 by March 14, 2011.

Check-in is after 3 pm and check out is prior to 11 am. Room and rate availability cannot be guaranteed after March 14, 2011.

Over 250 physical therapists, physical therapist assistants, students and guests from throughout Wisconsin are expected to attend the WPTA 2011 Spring Conference in Green Lake. Though there is no onsite exhibit hall at our Spring Conferences, we invite you to participate through one or more of the following ways.

Sponsorships- Increase your visibility at the conference by sponsoring a breakfast, break, or even our Business Meeting or Awards Dinner. Your company will be receive
signage at the sponsored event along with an acknowledgement in every participant’s
conference packet. Sponsorship fees: Thursday or Friday Morning and Afternoon Breaks are $150; Thursday, Friday or Saturday Breakfast is $150; Friday Business Meeting
Luncheon is $300; Awards Dinner Sponsorship is $300.
Advertising- Advertising will be accepted for placement in every participant’s conference packet. Charges: Full Page-$250. Camera ready ads must be received by March 25, 2011.
Mailing Lists- A mailing list can be purchased with names and addresses of conference attendees. Pre-conference lists are $35 and will be sent to you two weeks prior to the
conference. Post-conference lists are $50 and will be sent to you within one week of the conference conclusion.
Material Distribution - WPTA can distribute your brochures/catalogs, giveaways, etc. to
participants for a $100 fee. Details on where to ship materials will be included with your confirmation.
