New Affordable Care Act tools offer incentives for providers to work together when caring
for people with Medicare
People with Medicare will be able to benefit from a new program designed to encourage primary
care doctors, specialists, hospitals, and other care providers to coordinate their care under a final
regulation on Accountable Care Organizations issued today by the Department of Health and
Human Services (HHS).
On October 20, 2011, HHS announced final rules and a new opportunity for financial support to
help doctors, hospitals, and other health care providers work together to improve the care for
Medicare patients. By choosing to become Accountable Care Organizations, providers will be
able to share in savings by better coordinating patient care, providing high quality care, and
using health care dollars more wisely.

Wisconsin Medicaid Update for Membership March 4, 2011

Throughout the years, the WPTA MA Committee has worked on behalf of the membership on such issues as:
- Payment Rate reform
- Daily Encounter Documentation Requirements
- Revision of the Prior Authorization Therapy Attachment (PA/TA) and Spell of Illness Attachment (SOIA) documentation forms
- Ongoing clarification of documentation expectations related to PAs
- Development of the Therapies Handbook language and subsequent clarifications through Medicaid Updates
- Transition to Forward Health and the Forward Health interchange system for billing and prior authorization
- Suggestions for enhancing the list of diagnoses appropriate for use of the SOIA
- Clarification of the relationship between Wisconsin Medicaid and the Wisconsin Birth-to-Three program
- Rate Reform 1.0, 2.0 and most recently 3.0; offered suggestions to DHS on cost-saving options during each of these discussions among many stakeholders
These are just a few of the activities the committee and WPTA leadership have embarked upon in the last 10 years.
In the last few weeks there have been many indications that the Wisconsin Medicaid program will be undergoing significant changes in the coming months and years. The WPTA Medical Assistance Committee in consort with WPTA leadership and Jason Johns, WPTA Lobbyist, have continued to not only monitor this situation but have been directly engaged in discussions with the State Medicaid Director, Brett Davis. We are currently working very closely with the Department of Health Services (DHS) and the Medicaid Director, having had a face-to-face meeting with him on February 17, 2011. At that meeting, we presented three proposals outlining our concerns and suggestions for the program as it relates to physical therapy services. Much of the information in those documents reflects the voices of WPTA membership over the past several years. As a result of that meeting, the WPTA has been offered a place at the table for discussion of Prior Authorization reform with other stakeholders. We were also assured that across-the-board Provider Assessments was “off the table” at this time and this was reflected in the 2011-2013 State Biennial Budget just released March 1, 2011. The proposals we discussed at the meeting with Brett Davis can be found on the WPTA website’s Reimbursement page, Medicaid News. Also on the website, you will find a letter from our President, Kip Schick, written to Brett Davis, State Medicaid Director, confirming our commitment to work collaboratively with him and his department on initiatives that will create efficiency while preserving efficacy and integrity within the Medicaid program.
The WPTA Medical Assistance Committee remains your source for answers to MA-related questions. We will continue to monitor and respond to changes within the MA program that affect physical therapists and their patients, keeping in mind that our role is to represent, as best as possible, ALL WPTA members.

Attention Medicaid Providers:
Wisconsin Medicaid is embarking upon another round of cost-saving measures and needs your input. On November 11, 2010, WPTA will be involved with other provider groups at a meeting with DHS to discuss further ideas to reduce costs within the Medicaid program. You are encouraged to read about the current plans and to submit your own ideas.

Here, you can access a copy of the power point presentation and also submit ideas to improve health care outcomes and generate Medicaid savings prior to the breakout session on November 11.
Of interest is a proposal to implement an across the board provider assessment which would entail up to a 2% tax on gross revenues. The funds obtained through this tax would be redistributed in part as increased Medicaid payments to providers. The remainder of the tax revenues would be used to enhance the current Medicaid budget which is running a deficit this biennium.
